Why does clicking on the 'View First Unread' icon hardly ever do just that? Sometimes it does but is mostly fairly random.

I can give you a complete and definitive answer on this (I think): it appears there has been a bug on this for some while and it isn't (again, apparently) considered high enough priority to fix it ...
Advice: don't bother with it
